For all appointments and inquiries, please call our South Bend office at 574-968-2832. You can also request an appointment through the Patient Portal by clicking HERE

Dr. Mahon generally sees patients in South Bend all day Monday and Wednesday as well as Friday mornings. He sees LaPorte patients twice a month on Thursday mornings.

NEW PATIENTS

Please arrive 15 minutes early to fill out the new patient paperwork. Please bring the following to your first appointment:

Current insurance card
Current photo identification
A copy of any referral required by your insurance company
Copies of any previous medical reports, x-rays, MRIs or other diagnostic imaging
A list of your current medications

RETURNING PATIENTS NOT SEEN FOR 1 YEAR OR MORE

Please arrive 15 minutes early to update paperwork. Please bring the following to your appointment:

Current insurance card
Current photo identification
Updated medication list

TO CANCEL AN APPOINTMENT

Please call 574-968-2832 and provide 24 hours notice if you need to cancel your appointment. Two or more missed appointments or rescheduling/canceling with less than 24 hours notice will incur a $30 charge that is not payable by your insurance company.
